At Hazeltree Care Services we offer a specialist supported living service to service users who have been identified by the Local Authority as presenting with a Learning Difficulty, Autism, or a Mental Health condition.
​
We also support other groups from 18 to 65+ such as those with or without sensory impairment, mobility issues and dementia etc., but who are capable of living independently in their own homes.
​
This care can be long term but we do offer short term and recuperative care, as well as respite care where needed.
We also work with Case Managers and Physiotherapists supporting service users who have undergone life changing trauma accidents to reach their recovery goals and personal targets.
We Support adults aged 18 to 65+ whom have one or a combination of the following:
