WELCOME TO HAZELTREE CARE SERVICES
We are a Support Company with a team of professionally trained support workers who offer person-centred quality support to individuals within the borough of Milton Keynes and all surrounding areas. All of our clients are treated as individuals, with their own needs and support requirements, therefore all support given will be tailored to the exact need of the person. By doing this we can ensure that your loved ones will receive the highest quality of support possible. We strive in the fact that we listen to the needs of our clients in order to ensure their voices are heard. We also work closely with people in the community such as GP's, Hospitals, Clients families etc., to customize options that will help them achieve their day to day goals.
We work with Case Managers and other professionals where a service user's needs requires working as part of a multi-Disciplinary Team (MDT) to meet their best interests.
Please note that we currently do not provide personal domiciliary care, as part of our service. We hope to be able to offer this service by summer 2025.
